Complete your Family Income Inquiry Form Online

As you may know, we ask families to fill out a Family Income Inquiry Form each school year. The Family Income Inquiry Form is a key factor in determining whether your child’s school is eligible to receive Title I and other funding.  For example, Title I provides resources for programs and services to eligible schools.  By submitting this form, you can help ensure that funding for your child’s school is properly calculated for these resources.

We encourage you to complete a Family Income Inquiry Form online over the summer so that you will not need to submit a paper form in September.

Beginning in July, you can complete a Family Income Inquiry Form online at myschoolapps.com which is quicker, easier and more accurate than filling out a paper form.  Online forms are submitted using your student’s 9-digit ID number. You will receive immediate confirmation that your form was submitted.

Your privacy is important to us. All information is kept confidential. Federal regulation do not allow the DOE to share your information with any other agency.
